Will the Avalus Cows Aortic Valve Replacement Last A Lifetime?
📍 Introduction:
If you've been told you need aortic valve replacement surgery, you face an important choice. Do you choose a mechanical or a tissue valve? In this video, we'll explore the pros and cons of each option, focusing on the Medtronic Avalus pericardial valve and its potential benefits.
📍 Mechanical Valves:
Mechanical valves, including the OnX carbon valve, can last a lifetime but require lifelong use of blood thinners like Warfarin. This medication necessitates regular blood tests and increases bleeding risks if injured.
📍 Tissue Valves:
Tissue valves, made from pig or cow tissue, don't last forever but have significantly improved. Modern examples include the Inspiris Resilia valve by Edwards Lifesciences and the Medtronic Avalus pericardial valve.
📍 Medtronic Avalus Pericardial Valve:
Today, we're focusing on the Avalus pericardial valve. This stented valve, treated with Amino Oleic Acid (AOA) to prevent calcification, has shown no structural deterioration at 5 years in the PERIGON trial.
📍 Long-term Durability:
Early evidence suggests the Avalus valve could last 10-20 years, comparable to the Inspiris Resilia valve. Its excellent hemodynamic function means less obstruction to blood flow, potentially offering better performance.
📍 Age Considerations:
Under 50: Mechanical valves like the OnX carbon valve are recommended.
Over 70: Tissue valves are generally preferred.
Ages 50-70: Both options are viable, with lifestyle and durability being key factors.
📍 TAVI (Transcatheter Aortic Valve Implantation):
If a tissue valve fails, TAVI can replace it without surgery, offering a less invasive solution.
📍 Minimally Invasive Surgery:
Modern surgery techniques allow valve replacement through a small cut between the ribs, leading to faster recovery and preserving the breastbone.
📍 Conclusion:
Choosing the right valve depends on various factors, including age, lifestyle, and medical history. Both mechanical and tissue valves have their benefits and potential drawbacks.
For more information on the Inspiris Resilia valve, check the linked video at the end. We'll also be publishing a video on the OnX carbon mechanical valve soon. Like and subscribe to our channel for updates!

There are equivalent valves for the mitral Position but the data is early and we await longer term verification of durability. If your mitral valve can be repaired then this is the better solution to replacement

The data still suggests that mechanical is the preferred choice in young age. Having said this your comments are very good and many younger patients are preferring to follow tissue valve by mini surgery between ribs, and TAVI or traditional mini breastbone repeat surgery in later life depending upon clinical suitability

3:04 I had this valve fitted in Feb 2021. It was still in the clinical trials phase. Nice that I wasn't even asked to participate, just found out 4 weeks after surgery. I had been asymptomatic prior to surgery but post surgery, I was hit with multiple symptoms from dizzyness, constant fatigue, breathless and even fainted. So much for a advanced valve. If Edwards want their donkey valve back, they can take it any time. My experience is nothing but negative. I wish that I never gave my ill informed consent.

Hello doctor. I am a 29 year old male. I am 180cm tall and 89kg. This year i was diagnosed with a dilated ascending aorta at 4.2 cm. At first my doctor told me its normal amd that its because of bodybuilding and heavy weights. I did not like the response i got and i went to another doctor. He did an echogram and told me that i had an biscupid aortic valve which for the moment works fine and that caused my dilation. He also recommended a ct scan. I did one which showed my ascended aorta at 4.16 cm with no calcifications and no sign of disection. The doctor told me to excercise reguraly(running, sqimming, cycling, walking and moderate lifting). I have 2 queations 1- is it considered aneurysm at 4.2? 2- is having sex dangerous because it can spike your blood pressure for a certain amount of time?

7 1/2 years ago, a CT scan to find my calcium score (which was 0) showed a mildly dilated aortic root of 3.8cm and after adjusting for my height, weight and body mass, was told I am simply at the "outer limits of normal". This was confirm with a follow up CT scan one year later, this time with and without contrast and stable, not growing. I was told we can just follow with echo every 2,3 years. Just had a echo last week, and after 7 1/2 years, still stable at just 3.8cm and was told this probably is just normal for me and very unlikely I would ever need any surgery. Rest of echo, completely normal. Blood pressure is normal at 115/76. Lipid blood work is optimal. Do you agree?

Last year I had a cardiac score test performed. Not needed, the local hospital had a special for heart month, and I just wanted to see how I scored. It was perfect! They did notice that my ascending aorta was dilated at 4.4 cm. My doctor said it wasn't of concern. At this years physical I reminded my doctor so she decided to send me for another scan to see if there were any changes. I just got my results back today and the number has not changed. Still 4.4 cm. I haven't spoken to my doctor yet about the results, but when she gave me the scan referral earlier this year, she said I could have been born with this. Is this true? Can people be born with this condition? For reference I'm a 48 y/o female. No family history of heart conditions.

Can fluid retention be controlled through a diuretic?

  • @stevepfi1
    @stevepfi1 2 місяці тому

Fluid retention is the biggest problem with a heart valve replacement. If you have a heart valve replacement, you will have some level of fluid retention which makes it hard to breathe. So, the cardiologist will recommend torsemide which works great. It is amazing at making breathing easy again. The problem is they don't prescribe a supplement to replace the electrolytes lost through urination. You will definitely need two supplements: potassium and magnesium. Interested to hear what The Keyhole Heart Clinic has to say... and what about CoQ10?

    • @Thekeyholeheartclinic
      @Thekeyholeheartclinic 2 місяці тому

Thanks for your valuable comments. Frumil is a combination diuretic containing frusemide and amiloride which is potassium sparing and very useful. Magnesium deficiency after cardiac surgery often necessitates supplementation but under supervision of serum magnesium levels usually. CoQ10 is most useful in helping to counteract muscle pains in people on statins who suffer side effects. Hope this helps.

It is important to know that your palpitations are not atrial fibrillation. Also in borderline cases of mitral leak it is worth considering an exercise echo. Second opinions are always valuable.

This is furring up of the vessels and is a sign of generalized artery disease. This is not a good sign and requires risk factor control: blood pressure, blood sugar, cholesterol, diet etc

8.6cm aorta is extremely large and are at risk of rupture or dissection.

Can anyone take Ozempic for weight loss

In the UK the medication is restricted to morbidity related weight control. In the USA, I believe l, many celebrities take it for weight control! As a physician I do not support the latter use until we understand the long term effects of the drug

I need you as my mentor... I have a bicuspid valve and an aneurysm. I'm measuring at 47mm and they say when I hit 50mm I will need surgery. I'm so concerned in doing anything aggressive at the moment just incase I make it worse. Can they do a keyhole surgery on me when the time comes?

just got my results today and the dilation is 4.5 cm. the pain in my top middle back I describe as a deep very painful toothache, it never goes away. I am 57. My Dad and my Mom's sister (my Aunt) have both had open heart surgery, but I don't know the original reasons. I am a little frightened. I am stoic tho.

I had open heart surgery to repair a dilation was 5.4. Had the valve replaced also. Was 60 at the time and am doing very well. It’s a scary surgery but ensure you have a trusted hospital and surgeon and you should get through it well.

Oprah says it should be taken for LIFE. Everyone has GLP-1 agonists in their body. But the GLP-1 agonists in the bodies of obese ppl don’t work right. So STOPPING the med is dumb. No amount of counseling about diet and exercise will make a difference, and NEVER HAS for those with GLP-1 agonists that don’t work right. Don’t you thing obese ppl have already tried every method known to man?! They don’t WORK for them bc their GLP-1 agonists aren’t working right. Ins cos / NHS shouldn’t mind paying for these meds for life. It’s cheaper than paying for all the medical issues obese ppl develop. It’s cheaper than Bariatric Surgery. Many ppl can get their meds from a compounding pharmacy through a NATUROPATH if their drs won’t rx it or if ins won’t cover it. They then pay out of pocket.
